Licensed Vocational Nurse - ACT
The Licensed Vocational Nurse (LVN) for the Assertive Community Treatment Team (ACT) will be responsible to manage and lead the ACT team in providing crisis care, case management service delivery, individual rehabilitation activities, skills building, transportation and all aspects required to help clients succeed. The incumbent will assist adults residing in Salinas and South Monterey County with serious mental illness and serious functioning impairments, new to services, not engaged with services, and/or difficulty connecting to system’s services due to psychosocial and/or other barriers. The LVN will function as a coordinator of patient care for the ACT team, working closely with the Registered Nurse to provide culturally linguistic appropriate services while collaborating with other care providers and health team members to provide required care. Helps coordinate medication management and education. The individual will maintain accurate and secure summary information based on documentation in computer files and databases.

Division Director of Program Services
Required: Bilingual in Spanish. Licensed from “approved” vocational nursing course and/or completion of equivalent education and experience requirements for licensure examination approved by the California Board of Vocational Nursing and Psychiatric Technicians. Knowledge of principles, methods, and procedures of modern nursing care; first aid procedures; basic life support (BLS) and cardiopulmonary resuscitation; physical and mental health assessment techniques; basic anatomy, physiology, and medical terminology; EMR and electronic documentation; techniques of administering medications and treatments; protocols for issuance of referrals, medication refills, and general communications from Practitioners; clinical equipment and supplies; isolation techniques; sterile and aseptic techniques and protocol.
Preferred: One year of experience in psychiatric nursing. Knowledge of Medi-Cal documentation requirements and/or Quality Assurance procedures. Knowledge in the areas of substance abuse desirable. Proficient in Outlook email correspondence and computer software’s. Ability to; work as a member of a team, accept supervision, maintain appropriate boundaries while working in close inter-personal relationships; learn and apply principles of social rehabilitation; organized, responsible; flexible; empathic and able to relate effectively with clients, other staff, professionals and members of the community. Knowledge of community resources.
Valid California driver's license; auto in safe operating condition; auto liability insurance; good driving record; criminal record clearance; proof of authorization to work in the United States as required by Immigration and Reform Act of 1986. Ability to work independently with minimal supervision.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Under the supervision of the Program Director, the Licensed Vocational Nurse will assist with the following duties:
1. Provide on call services.
2. Provide culturally linguistic appropriate, recovery oriented, trauma informed, wellness centered services focused on nursing support for patients in homes and other environments.
3. Assist clients with self-administration of medication; knowledge of medication classification, interactions, expected therapeutic responses, and nursing implications on an individual basis or in a group setting.
4. Administer intramuscular injections to residents. Administer TB tests and document results.
5. Coordinate and consult with psychiatrists and other physicians regarding client health issues, medication interactions.
6. Provide general healthcare education to staff and residents on issues such as diabetes, cholesterol levels and high blood pressure, hepatitis, TB and HIV/AIDS, infection control procedures, management of lice, scabies, etc. Provide plans for infection control when a resident has a communicable condition.
7. Facilitate linkage with doctors and pharmacy and provide advocacy as needed.
8. Obtain and coordinate resources on nutrition training, diet planning, smoking cessation, and other health related issues as they arise. Facilitate groups on these topics according to resident need and request.
9. Assist RN and psychiatrist/Psychiatric Nurse Practitioner with patients’ medication education and knowledge of medication.
10. Provide culturally and linguistically appropriate recovery-oriented, trauma informed, wellness centered approach to consumers with substance use disorders using a harm reduction approach.
11. Provide services in differing environments, such as consumers’ homes, schools, gathering places, homeless shelters, etc. including community-based outreach and engagement.
12. Assist clients in attaining community stability and reaching their health, recovery and rehabilitation goals, including helping clients to find and keep employment.
13. Provides health education and support to clients and families.
14. Assist clients with their physical and mental health recovery process by developing the skills necessary to lead independent or interdependent, healthy and meaningful lives in the community.
15. Provide psychiatric and other education and advocacy to patients, patients’ families and other support members as appropriate.
16. Provide community-based outreach and engagement in homes and other environments.
17. Obtain and coordinate resources on nutrition training, substance use, diet planning, smoking cessation, and other health related issues as they arise. Counsel clients individually to set, track and achieve rehabilitation and recovery goals.
18. Maintain legible, accurate and timely documentation of treatment plans and other necessary information on clients, including documentation required for Medi-Cal standards.
19. Maintain accurate and secure summary information based on documentation in computer files and databases and follow the HIPAA Privacy Rules guidelines for protecting client’s medical records.
20. Identify, assess and respond to crisis situations. Set clear limits and ensure that both clients and facility are safe.
21. Document services and meet and record productivity and outcome standards of the program.
22. Travel and work at various program locations. (Marina, Monterey, Salinas)
23. Collaborate and coordinate services with multidisciplinary staff team members, i.e. Psychiatrist, Nurses, Family Partner, Peer and other staff to provide effective services.
24. Other duties as assigned.
Ability to: operate a motor vehicle; climb stairs; possess good communication skills; meet the requirements of the classification and have mobility, vision, hearing and dexterity levels appropriate to the duties to be performed. Capable of the following extended activities: standing, sitting, climbing stairs.
Capable of the following intermittent activities: lifting 25 pounds or more and bending in the performance of infrequently performed office duties.
